Physical Therapist - Home Health - (PT - HH)
StartDate: 3/10/2025
Available Shifts: 8 D
Pay Rate: $2374.17 - $2601.48
Physical Therapist - HH needed asap for 13 weeks
Schedule: 40 hours per week, Monday-Friday and one weekend a month
rotation
--- Required to prioritize start of care visits
--- Required to supervise Physical Therapy Assistants
--- Next day mandatory reporting of start of care visits
--- Clinical notes and other documentation due within 48 hours of
visit
--- Maintain timely communication with supervisor, intake, and
office
--- Be generally responsive during normal business hours
Requirement: CA PT License and one year of Home Health
experience
Territory: Marin, Sonoma, and Napa County, Ca (Will be required to
expand as needed)
Training: 3 full days, one in house and two in the field
Documentation System: Kinnser
Equipment Issued: iPad, Verizon One Talk
Mileage Reimbursement: $0.65.5/mile (from first to last
patient)
Parking and Toll Reimbursement: Paid
Company Car: Option with a decreased bill rate

be more than 30 days of gap in last 1 year
